Infection Control Investigations

This web page contains reports on premises where an infection prevention and control lapse (breach) was identified through the assessment of a complaint or referral, or through communicable disease surveillance. It does not include reports of premises which were investigated following a complaint or referral where no infection prevention and control lapse was ultimately identified.

These reports are not exhaustive, and do not guarantee that those premises listed and not listed are free of infection prevention and control lapses. Identification of lapses is based on an assessment and investigation of premises at a point-in-time. These assessments and investigations are triggered when potential infection prevention and control lapses are brought to the attention of the local Medical Officer of Health.

Reports are posted on the website of the board of health in which the premises are located. Reports are posted on a premises-by-premises basis, i.e. correspond with one site only. Should you wish to view a full investigation report for any posted lapse, please contact the Eastern Ontario Health Unit at 613-933-1375 or 1 800 267-7120.

Results will remain posted under “Reports” for 24 months from the date of the final inspection.

Please note: If you’re looking for an inspection report that is 2 years or older or not yet available on this website, or for a report of a premises that is no longer in business, please fill out a request form. The form is available for download here or at the EOHU office nearest you. You must fill one form per premises.

Submit a Complaint

Do you have a concern about the hygiene or safe practices of a particular establishment?

Contact the EOHU at 613-933-1375 or 1 800 267-7120 to submit an infection control complaint related to premises (establishments or facilities) in the City of Cornwall, SD&G or Prescott-Russell that:

  • Provide a personal service such hairstyling or barbering, tattooing, ear or body piercing, electrolysis, manicure, pedicure or aesthetics

OR

  • Are independent (non-hospital) health facilities

If you suspect you have an infection from receiving a service at a particular establishment or facility, consult with your doctor as soon as possible.
