Personal Service Setting Notification Form

The Eastern Ontario Health Unit (EOHU) is committed to working with new and existing personal service setting (PSS) owner/operators to ensure the health and safety of clients and employees.

In accordance with the Personal Service Settings, O Reg 136/18, personal service setting owner/operators must complete the form below at least 14 days in advance of the following:

• Opening a new personal service setting
• Expanding an existing service to provide additional personal services
• Reconstructing or renovating an existing personal service setting
• Relocating an existing personal service setting
