Preparing for Your COVID-19 Vaccination Appointment

On the Day of Appointment

On the day of your appointment:

  • Bring a piece of government issued ID showing your name, photo and date of birth (e.g. OHIP card, driver’s licence, passport). If you do not have a piece of government issued ID, let the clinic know upon your arrival.
  • Please arrive on time for your appointment.
  • Please report to the reception desk on arrival.
  • Have regular meals.
  • Wear short sleeves.

If you have symptoms of COVID-19 on the day of your appointment, please follow the instructions below and call the health unit to reschedule your appointment if you are required to stay home and self-isolate. If you are not required to stay home according to the criteria listed below, please present yourself to your appointment.

Stay home and self-isolate until all of the following apply:

  • your symptoms have been improving for at least 24 hours (or 48 hours if you had nausea, vomiting and/or diarrhea)
  • you do not have a fever
  • you have not developed additional symptoms

The COVID-19 vaccines may be given at the same time with other vaccines, or at any time before or after other non-COVID-19 vaccines (live or non-live vaccines), including influenza vaccine, respiratory syncytial virus (RSV) vaccine and/or the RSV monoclonal antibody, Beyfortus.
